Community Health Assessment Survey

Do you live in the South Suburbs of Chicago? We would like to hear from you for our Community Health Needs Assessment!

Help us understand what health problems affect the community by taking this quick survey.

This survey will only take about five minutes and asks about the health needs of our community. Please encourage your south suburban friends and family, particularly those living in Thornton Township, to take the survey, too.

Email DonElla Bradford, donella.bradford@uchicagomedicine.org
or Christine Ratliff, christine.ratliff@uchicagomedicine.org with questions.
